SUBSTANCE USE SERVICES
GROUP COUNSELING
Participate in group therapy with themes like relapse prevention, anger management,
mindfulness, and other topics determined by the needs of the people currently served.
INTENSIVE OUTPATIENT (IOP)
Counseling is provided in group treatment formats for a half a day program at least three
times a week. The IOP will focus on Behavioral Health or Substance Use needs depending
on which group may be the most helpful right now. Please call to determine eligibility.
RECOVERY COACH
Meet with your assigned Coach to receive one-on-one peer support. Coaches will
provide guidance and help you overcome the obstacles making progress difficult.

Family Counseling
Recovery success will result in change among family systems. Including them
in treatment may facilitate and support progress made in other venues.

Medication-Assisted Treament
As part of a holistic treatment plan, Medication-Assisted Treatment is a tool to support recovery.
Several medication options are available and can be discussed with your provider.
SOAP
The Structured Outpatient Addiction Program (SOAP) is available to people who have recently entered recovery or who are in
need of additional support to maintain their recovery. Counseling is provided in group treatment formats for half a day.
Recovery Coach

guidance and help you overcome the obstacles making progress difficult.
Recovery Support Navigator
Receive one on one case management support from a Recovery Support Navigator
specializing in knowledge of the Substance Use Treatment System.
Driver Alchohol Education & Second Offender Aftercare
Psychoeducation groups provided to fulfill requirements for court-referred individuals following a DUI Conviction

Northeast Family Services of New Hampshire, Inc.
Northeast Family Services (NFS) provides home or community based, wraparound, treatment for children and parents in need of services, with an intensive focus on keeping families together or working towards reunification. Our services include individual and family counseling, skill building, support, advocacy, and resource connection, to help manage the family’s future functioning in the home and community. Northeast Family Services prides itself in providing high quality, evidence-based treatment to families.
Roadmap To Reunification
Bringing everyone together to build relationships that respect birth parents, offer compassion, and encourage regular visitation and communication with the birth family.
Roadmap to Reunification facilitators bring all family members, supports, placement providers, and team members to the table when a child is removed by DCYF.
The goal of Roadmap to Reunification is to identify what tasks need to be accomplished in order to:

What to Expect:
A Roadmap to Reunification specialist will contact you to schedule an initial "Ice Breaker" meeting within 10 days of the removal of the child/children. Following the Ice Breaker meeting, if reunification has not yet occurred, you can expect check ins every 30 days (by phone or in person). After 6 months, the Specialists are available to consult on cases that need continued support. One specialist is assigned to every DOYF district office.
Ice Breaker Meetings:
The Ice Breaker is a tool, facilitated by the Roadmap Specialist, during the first meeting between the birth parents and the caregiver that is designed to build relationships and allow the birth parents to share crucial information about their child. Ice Breakers can only occur if both the birth parent and caregiver are present.
Roadmap Specialists will support conversations regarding:

Parenting time between birth parents and children/youth

Relative connections and supports for children/youth

Any barriers to reunification
Intensive In-Home Services
(Provider for FAST FORWARD)
Many New Hampshire children, youth, and families who experience difficulties in day-to-day life due to serious emotional disturbances end up being moved out of their homes. They are often put into residential treatment facilities, psychiatric hospitals, juvenile justice facilities, or daytime programs that take them out of their schools and communities. Despite the best intentions and hard work of families and providers services are difficult to navigate and are fragmented into uncoordinated systems. New Hampshire has begun to make big strides to address these challenges with a new approach. The FAST Forward program – Families and Systems Together – is designed to serve youth with serious emotional disturbances and their families, whose needs aren’t met by traditional service streams and programs.
ISO Program
(Individual Service Option)
ISO cases include a masters or bachelors level clinician and a bachelors level case manager. ISO cases include multiple sessions per week, both individual and family counseling, case management and supervised family time when appropriate. Frequency depends on request from DCYF but will include multiple days per week with flexible hours. NFS staff will complete a comprehensive assessment and the Child Adolescent Needs Survey (CANS) to guide an individual treatment plan on each case. ISO families have access to NFS’s after hours on call phone number in case of crisis. At this time, NFS does not have ISO foster homes so if respite is needed, NFS staff will work closely with the family to identify natural resources and with DCYF to identify a respite home and support the child/youth while in respite. In addition to traditional ISO cases, NFS also offers ISO specialized budgets for Independent Living Cases for youth aging out of foster care.
HBT Program
(Home Based Therapy)
NFS provides master’s level clinicians or bachelors level clinicians with 5+ years’ experience working with children and families. Depending on the needs identify at referral, HBT may utilize a bachelors level case manager. Frequency depends on request from DCYF but will include multiple days per week with flexible hours. NFS staff will complete a comprehensive assessment and the Child Adolescent Needs Survey (CANS) to guide an individual treatment plan on each case. HBT families have access to NFS’s after hours on call phone number in case of crisis.
CHS Program
(Child Health Support)
NFS provides bachelor’s level case managers for child health support cases. Supervised family time including before, during and after parent coaching, intensive parenting education, support and encouragement, direct interactions with the children and families and resource connection. Frequency depends on request from DCYF but will include multiple days per week with flexible hours, including nights and weekends. NFS staff will complete a comprehensive assessment and the Child Adolescent Needs Survey (CANS) to guide an individual treatment plan for each family. CHS families also have the same access as all other case levels to NFS’s after hours on call phone number in case of crisis.
